Short-term Outcomes of Laparoscopy-assisted Orchiopexy as Initial Management of Cryptorchidism

Abstract
Purpose: The purpose of this study was to examine the clinical characteristics of cryptorchidism and to investigate feasibility, safety and advantages of laparoscopy-assisted orchiopexy (LAO) as initial management of cryptorchidism.

Methods: From January 2013 to October 2015, 102 patients underwent LAO for cryptorchidism. A retrospective study was conducted by reviewing medical records and short-term outcomes of LAO were analyzed according to age and location of the testicles.

Results: The status and location of the testes was confirmed in all patients during laparoscopy. Testes were mainly located in the inguinal canal (79.4%) and in the right side (53.9%). Patent processus vaginalis was the most common associated condition with cryptorchidism in both inguinal areas. According to age, location of testis, laterality, operation time and complications were not significantly different. According to the location of testis, age and laterality did not show any differences. In the vanishing testis group, closed processus vaginalis was significantly more frequent and operation time was shorter. Mean operation time was 19.9¡¾8.71 minutes and there was no open conversion. All patients could be discharged on the day of surgery. There were 4 cases of postoperative complications, of which one was testicular re-ascent. Short-term success rate based on absence of testicular atrophy was 99.02%.

Conclusion: LAO for the initial management of cryptorchidism is a feasible and safe procedure without increase in complication rate, operation time and postoperative hospital stay. Therefore, LAO is a possible surgical option for pediatric surgeon specializing in laparoscopy.
